Insurance need not be complicated. For your reference, here are some definitions of terms you may find in our contracts, procedures, and guidelines.
A licensed healthcare facility, which is appointed by Krungthai-AXA Life, to carry-out health checks for the applicant of insurance. All qualified doctors at our contracted hospitals are eligible to undertake health checks. With the health check form, our agents or customers do not have to pay up front.
Appointed Medical Examiner
A licensed doctor of medicine certified by Krungthai-AXA Life to carry-out a health check for an applicant of insurance.
Medical Technician Responsible for Anti-HIV Tests Involving Blood/Saliva Samples
A licensed doctor or medical scientist appointed by Krungthai-AXA Life to collect blood and saliva samples for Anti-HIV, who is able to sign the consent form to have the test in affirmative consent part.
Health Check Form
Issued by Krungthai-AXA Life that allows an applicant of insurance to undergo health checks at Krungthai-AXA Life’s contracted hospitals without up-front payment.
Health Check Report
A document completed by a doctor or medical professional giving information about the state of health and physical examination results of the applicant for insurance.
1. Health check for a child aged under 16 needs to use form UW04. The front of this form is used to record the health of the child and the back is used to record the health of the policy premium payer (who must be the child’s legal guardian).
2. Health check for anyone aged over 16 needs to use form UW02. The front of this form is used to record medical history of the applicant for insurance and the back is used to record the applicant’s general health information including weight, height, urine test, and other tests as requested.
