    Noted In case of any insurance requires that the applicant for insurance undertake a proper health check, the agent will take the applicant to a hospital for the checkup.

  • Application Process

    Applying for an insurance is an important first step towards protecting your financial future. We try to make it as easy as possible so you can go on with your regular activities with greater peace of mind. Here are some important things you should know:

    1. Choose your preferred base plan, riders, sum insured, and premium;
    2. Fill-out the application form completely and attach all the required documents;
    3. Submit the completed application form and required documents and pay your premium via an agent or at our branch office, whichever suits your convenience;
    4. You will receive a temporary receipt from the agent or a personnel from our branch office;
    5. Await the delivery of the insurance policy from your agent;
    6. If the review of your application is not concluded within 30 days after the receipt of all documents required and premium payment, Krungthai-AXA Life will send a notification to the address given in your application form;
    6. Sign to acknowledge your receipt of the insurance policy and check for correctness. If there is any missing or incorrect information, please contact your agent or branch office right away.

    *Branch office means general agency (GA) office or Krungthai Bank branch.

  • Consideration Period

    Consideration period depends on information given and the required documents. Details are as follows:

    - If you have submitted all documents required, it normally takes no more than 30 days after you have paid your premium;
    - If you are required to provide additional documents, it will take no more than 30 days after receipt of all additional documents required;
    - If the review of your application is not concluded within 30 days after receipt of all documents required and premium payment, Krungthai-AXA Life will send you notifications from time to time until the review is completed.

  • For your convenience, we offer a wide variety of payment options:

  • Payment by Credit Card

    You may also choose to pay your premium via credit card. We accept VISA, MASTER CARD, JCB Card and AmEx (American Express)

    - A "Credit Card Premium-Payment Authorisation Form” must be filled out before payments by credit card can be made. The credit card owner must be related to the policyholder e.g. father, mother, son, daughter or spouse. (Contact your agent to notify credit card payment.)
    - The credit card number must be filled in accurately and clearly.
    - The "Credit Card Premium-Payment Authorisation Form" must be signed with the same signature as the one on the back of your credit card.
    - Copy of either the credit card or ID card of the credit card’s owner must also be attached.
    - The completed form and all required documents are to be sent to

  • Payment via Krungthai ATM Card

    Premiums can also be conveniently paid at any time, 24 hours a day, with your Krungthai ATM Card at any Krungthai ATM machine nationwide. Daily limits are, however, varied depending on the type of ATM card you have. Details are as follows:

    - Classic Card 20,000 Baht/ day
    - Gold Card 50,000 Baht / day

    The bank will also charge you a service fee for making the payment:

    - Bangkok and vicinity 10 baht / transaction
    - Other provinces 20 baht / transaction

    You may also make payments at Counter Services at 7-Eleven or Tesco Lotus (24-hour services).
    The following are the details:
    - For Counter Services at 7-Eleven, payment amount must not exceed 30,000 Baht.
    - For Tesco Lotus, payment amount must not exceed 49,000 Baht.

  • Payment Over Bank Counters

    Payments may be made at any of the following banks:

    - Krungthai Bank
    - Bangkok Bank
    - Bank of Ayudhya
    - Kasikorn Bank
    - Siam Commercial Bank
    - TMB Bank
    - Siam City Bank
    - Bank for Agriculture and Agricultural Cooperatives

    Payment can be made as follows:
    - By cash, for both first-year premium and renewal premium;
    - By cheque, for both first-year premium and renewal premium (except renewal premium with the invoice code AC07142 of which payment is required to be made by cash); or by bank draft or cashier’s cheque made payable to “Krungthai-AXA Life Insurance PCL” with the word “or holder” crossed out

  • Postal Payments

    - In making postal payments, the bottom half of your insurance premium notice invoice AC08142 should be enclosed and sent to Krungthai-AXA Life together with your premium, which can be paid as follows:

    - Cheque or bank draft made payable to “Krungthai-AXA Life Insurance PCL.” with the words “or holder” crossed out;
    - Postal order stating “Krungthai-AXA Life Insurance PCL.” and made payable to P.O. Nana Nua, Bangkok;
    - Cashier’s cheque made payable to “Krungthai-AXA Life Insurance PCL.”
    Please do not enclose any cash

  • Payment via Agent

    Premium can be paid either in cash or by cheque. Your agent will issue you a valid temporary receipt immediately after you make your premium payment.

  • Payment by Direct Debit

    Premium payments can be conveniently made by direct debit from either your savings or current account with any of the following banks:
    - Krungthai Bank
    - Bangkok Bank
    - Bank of Ayudhya
    - Kasikorn Bank
    - Siam Commercial Bank

    Payment by direct debit can be made either monthly, quarterly, bi-annually or annually. A Direct Debit Request Form must be filled out so that the bank can verify the account number and the account owner’s signature. (This form can be obtained from either Krungthai-AXA Life head office, any branch offices, or general agency offices) with the following documents:
    - Direct Debit Authorisation Form
    - Copy of the first page of your bank book

  • Maintaining your Policy

    - This policy is a contract of insurance between you (the insured) and Krungthai-AXA Life (the insurer). Krungthai-AXA Life is responsible for providing protection with guaranteed benefits for you throughout the duration of the policy. Meanwhile, your responsibility is to keep the policy active and pay your premiums on time throughout the agreed period.

  • Invoicing Procedure

    - You will be sent an invoice for your insurance premium 28 days before the payment due date.
    - If you fail to pay your invoice on the due date, an overdue notice will be sent to you 7 days after the due date.
    - Krungthai-AXA Life allows 31 days of grace period after the payment due date, and you are still covered.

  • Renewing your Policy

    Once the grace period is over, Krungthai-AXA Life will automatically proceed according to regulations of the policy (if the policy has adequate cash value). If your policy is invalid, you can contact your agent or Krungthai-AXA Life in order to request a renewal of your policy.

    Policy renewal request can be made within 5 years from the expiry date by contacting Krungthai-AXA Life or your agent. You will also have to submit additional documents for review.

  • Having Difficulty Making Payments

    If you have difficulty making your premium payments, we offer you the following solutions:
    - Reduce sum insured
    - Change your premium payment period (annually, bi-annually, quarterly, monthly)
    - Cancel rider(s)
    - Change insurance coverage
    - Convert to paid-up policy or extended term insurance. If your policy is 2 years old and over or generates cash surrender value, it can be converted either paid-up policy or extended term insurance.
    - Paid-up policy allows the policyholder to enjoy coverage for the originally agreed term period but there may be change in sum insured (depends on policy year and insurance plan).
    - Extended term insurance is another option of which sum insured remains the same (will be deducted if there is any policy debts) while the coverage will be extended for a period of time. If the insured dies during the extended term, the beneficiary will receive the sum insured. If the insured lives through the end of the extended term period, net accumulated income will be paid (if applicable).

    Note: Extended term insurance is only applicable to policy with standard premium rate.

    - Request a policy loan. A policy loan can be requested if you have paid premium for 2 years or more or your policy generates cash surrender value. The maximum amount of loan is 90% of the policy’s cash value, with interest rate of not exceeding 8% or more per annum.

  • Change of Correspondence Address

    Inform Krungthai-AXA Life in writing

    Required documents:
    - Service request form

  • Change of Name / Last Name / Signature of Insured

    Inform Krungthai-AXA Life in writing and sign the service request form with your new signature

    Required documents:

    - Service request form
    - Official name/last name change certificate
    - Copy of ID card

  • Change of Beneficiary

    The new beneficiary must be an immediate family member.

    Required documents:
    - Service request form
    - Copy of household registration of the new beneficiary
    - Copy of ID card of the new beneficiary

  • Change of Premium Payment Period

    This could be changed at the policy anniversary date or within 31 days of the grace period

    Required documents:
    - Service request form
    - Premium payment

  • Health Check Procedure

    Both life and health insurance are directly concerned with your health background. To evaluate your risks (as an applicant for insurance), information about your state of health is required.

    There are two types of insurance:
    - Insurance without a health check
    - Insurance with a health check

    The key factors that determine whether it is insurance with or without a health check are: age, sum insured, medical history, and information stated in your application form.

    Complete and accurate health check results will clearly indicate your true state of health. Your medical history will greatly help us in our assessment of your application. It will also allow us to provide you the best possible service that meets your needs, avoid claims rejection, or contract termination.
