5.       Specify policy which you would like to declare your intention to exercise the right.

• Intension to exercise for all insurance policies that I am holding with Krungthai-AXA Life Insurance Public Company Limited.
• Intension to exercise the tax exemption right only for specific insurance policies
• No intention to exercise the tax exemption of all insurance premiums.

If you have previous declared your intention, the company will consider this notification as your latest intention.

1.       If you do not declare KTAXA of the intention to exercise the rights may make you unable to deduct the personal income tax according to the requirement of the Revenue Department.

2.       The above insurance policy is eligible for claiming tax exemption according to the tax laws, excluding insurance policies under consideration or pending approval.
